Adult Protection Planning Support Remote position with training in Warren, MN Dancing Sky Area Agency on Aging are looking for a self-motivated, innova- tive person that enjoys working on new projects. This position is time limited ARPA grant funded. We would be looking at supporting care planning work for older adults that need care support to reduce their risk and support safe independent living. Strong skills in planning, organization and follow through in a high paced environment. Strong computer skills. Travel and overnight stays required within 21 county service area. Specific responsibilities: Tracking of project and persons served Outreach and relationship building to launch project Support planning skills for individuals and families . . . Strong understanding of aging services County coordination and support in building referrals Other duties as needed to complete the pilot project . Experience Required: . Referred bachelor's degree from an accredited college or university in nursing, social work, gerontology, or related human services field. Experi- ence in care coordination and case management. OR a minimum of three years' work experience is preferred. Other requirements: Successful completion of criminal background check Grant funded temporary position 1 FTE. Position open until filled.
